A comparison of the effectiveness of three different fluoride regimens in increasing the acid resistance of enamel

Abstract
Objective: The effectiveness of topical fluoride as a cariostatic agent is well established. Although professional topical fluoride is used to arrest active caries lesions, there has been no comparison of the topical fluoride regimens on the acid resistance of enamel. This study compared the effect of acid resistance of enamel after fluoride application using three different fluoride regimens.

Methods: Forty-eight bovine teeth, 3 §® in diameter and 3 §® in depth, were demineralized. The following test groups of 12 specimens each were considered; (1) non-treated(control)(n=12); (2) Iontophoresis using 2% sodium fluoride solution(F-iontophoresis)(n=12); (3)1.23% acidulated phosphate fluoride gel(F-gel)(n=12); (4) 5% sodium fluoride containing varnish(F-varnish)(n=12). After the treatment, the specimens were mounted into the removable acrylic mandibular appliance. Six volunteers wore the appliance in their mouth more than twelve hours a day for four weeks. Fluoride electrode, inductively coupled plasma atomic emission spectroscopy(ICP-AES), electron probe microanalyze(EPMA) were used to analyze the increasing effectiveness of acid resistance.

Results: In F-gel group, there was a higher level of KOH-soluble fluoride, which appeared as a calcium fluoride-like layer, than the control. This also effectively contributed to increasing structurally bound fluoride. In addition, EPMA analysis indicated that calcium and phosphorous remarkedly deposited until the depth of 100 §­ in the F-gel group. ICP-AES analysis demonstrated that the F-gel group had the lowest etching depth and weight, and a lower level of calcium and phosphorous ion dissolution after acid treatment.

Conclusion: The F-gel had most beneficial increasing effect on the acid resistance of enamel over the other
